Diabetes, hipertensión y enfermedad renal crónica: una interacción epidemiológica clave en la transición de las enfermedades no transmisibles.

Resumen

INTRODUCCIÓN: La enfermedad renal crónica fue reconocida como una de las principales enfermedades no transmisibles emergentes a nivel mundial, con un aumento sostenido de su carga entre 1990 y 2023, según el Global Burden of Disease. La relación entre la diabetes mellitus y la hipertensión arterial la posiciona como un componente clave de la transición epidemiológica, especialmente en regiones de ingresos medios y bajos. EL OBJETIVO del estudio fue analizar la magnitud, la distribución y las tendencias de la enfermedad renal crónica, así como su relación con la diabetes mellitus y la hipertensión arterial, integrando evidencia clínica reciente de América Latina. METODOLOGÍA: se realizó una revisión narrativa y analítica de la literatura, basada en estudios poblacionales, informes del Global Burden of Disease, metaanálisis, consensos regionales y guías clínicas internacionales. La información recopilada se sintetizó para describir la carga de la enfermedad, los principales factores asociados, las brechas diagnósticas y los avances terapéuticos recientes. RESULTADOS: El análisis revela un aumento notable de la incidencia y la mortalidad asociadas a la enfermedad renal crónica (ERC) a escala mundial, caracterizada por pronunciadas desigualdades regionales. Se identificó una interacción sinérgica entre la diabetes mellitus, la hipertensión arterial y el deterioro progresivo de la función renal, que acelera la progresión de la enfermedad renal crónica y eleva el riesgo de complicaciones cardiovasculares. Asimismo, se evidenció un grado considerable de infradiagnóstico, atribuible a la baja utilización de biomarcadores tempranos de daño renal, en particular de la albuminuria. Aunque en los últimos años se han incorporado nuevas terapias con efectos nefroprotectores capaces de modificar la evolución de la enfermedad, su implementación en la práctica clínica sigue siendo desigual y limitada. EN CONCLUSIÓN, la enfermedad renal crónica constituye un desafío creciente para la salud pública que requiere estrategias preventivas e integradas, orientadas a la detección temprana, el control oportuno de los factores de riesgo y la ampliación del acceso a intervenciones terapéuticas basadas en la evidencia.
